OHF HEALTH SCREEN QUESTIONNAIRE
It is mandatory to ensure you are completing the Ontario Hockey Federation Health Screening Questionnaire prior to entering the arena before every practice or game.
PARTICIPANTS:
Please ensure you are pre-screening prior to arriving at the rink.
Once you arrive, your team manager will ask for a verbal confirmation that you have completed the self-screener prior to arriving.
